Why does the MegaRAID SAS controller complain about firmware faults?

Issue

  • Having problems with our storage backed by a MegaRAID SAS card. In /var/log/messages receiving messages about firmware being in a FAULT state. What can be done about this?

    kernel: megasas: Found FW in FAULT state, will reset adapter.
    kernel: megaraid_sas: resetting fusion adapter.
    kernel: megasas: Waiting for FW to come to ready state
    kernel: megasas: FW now in Ready state
    kernel: megasas:IOC Init cmd success
    kernel: megaraid_sas: Reset successful.
    

    or

    kernel: : megaraid_sas: FW detected to be in faultstate, restarting it
    kernel: : megaraid_sas: FW restarted successfully,initiating next stage
    kernel: : megaraid_sas: HBA recovery state machine,state 2 starting
    kernel: : megasas: Waiting for FW to come to ready state
    kernel: : megasas: FW in FAULT state!!
