File ownership on NFSv4 mounts set incorrectly with Red Hat Enterprise Linux 6.4 or 6.5

Issue

I followed the steps in article: NFSv4 mount incorrectly shows all files with ownership as nobody:nobody and the problem seems to be corrected for a short time but comes back within hours or days.
Issuing the command "nfsidmap -c" temporarily restores currently 'wrong' UIDs but there is no telling when or where the "nobody" ID will show back up. What is the permanent fix?

Environment

Red Hat Enterprise Linux 6.4
Red Hat Enterprise Linux 6.5 (kernel < 2.6.32-431.29.2)
nfs-utils-1.2.3-39 (must be > 1.2.3-36 and < 1.2.3-40)
