Multipath devices are not created at boot time
Issue
- The following errors are displayed in the logs at system boot
kernel: device-mapper: table: 253:27: multipath: error getting device
kernel: device-mapper: ioctl: error adding target to table
- Duplicate PV errors are being shown when running lvm related commands.
