Qlogic driver reports 'Failed mbx[0]=4005, ... cmd=31' errors

Issue

  • Qlogic driver shows following error messages while booting up the system, although there were no issues observed while accessing any SAN devices. Does this error messages indicates any issue with SAN/Storage? is this a bug?

  • QLogic HBA (including following models):
    • QLogic Corp. ISP2532-based 8Gb Fibre Channel to PCI Express HBA (rev 02)
    • QLogic Corp. ISP2432-based 4Gb Fibre Channel to PCI Express HBA (rev 03)
