leapp のアップグレードが "Cannot activate LVs in VG rootvg while PVs appear on duplicate devices" エラーで失敗する

Issue

leapp upgrade の再起動部分中に、システムが緊急モードで失敗します。コンソールログまたは rdsosreport.txt には、重複したデバイスに PV が表示され、LV のアクティブ化に失敗したことが示されます。
