Environment
- Red Hat Enterprise Linux 5,6,7,8
- Most common on FC adapters, but can occur on other types of HBA
- FC adapters: Emulex (lpfc), QLogic (qla*), or Brocade (bfa) Fibre Channel Drivers
- Other adapters: LSI (megaraid_sas), HP's Smart Array (hpsa), etc.
