Multipath is listing multiple child paths for one single path

Issue

  • Same port is being registered under a new scsi target index with different numerics.
  • QLogic HBA discovers the same port over and over again.
  • Repeated storage targets are seen through the same switch/SAN to the same end port .
  • Same target port shows up multiple times along unique hba->switch->storage path.
