RHEL 6 server with qlge driver experiences link failures, "LOOP DOWN" messages, path failures, I/O errors, and/or crashes when running sosreport or 'ethtool'

Issue

Running ethtool -S on a QLogic NIC using the qlge module causes the system to crash.
Attempting to generate an SOS Report with the sosreport command causes the system to crash.
I ran sosreport and my system was fenced after all of its paths failed
