Why are RHEV VMs pausing due to storage permission problems?

Issue

Unable to resume the VM from paused state
Other guests using the same storage are still operating normally
This was a VM imported by virt-v2v from VMWare ESXi 5 several weeks ago
The vm "intermed" has start successfully after the import and it has been running for several weeks without issue until now
All 3 disks for this VM were imported
No configuration changes after import

Red Hat Enterprise Virtualization (RHEV) 3.0
Red Hat Enterprise Virtualization Hypervisor release 6.2 (20120510.0.el6_2)
VMWare ESXi 5
