Server doesn't boot after creating new initramfs

Issue

  • The Logical Volume (LV) with the root filesystem is no longer found at boot
  • After patching the server it doesn't boot because the LVs where the OS is installed cannot be found.
  • After creating a new initramfs the server doesn't boot because the LVs where the OS is installed cannot be found.
  • Although the disk with the Volume Group (VG) containing the root LV is still visible within rdshell, the VG is not found by issuing vgscan

Environment

  • Red Hat Enterprise Linux (RHEL) 6
    • Veritas VxVM or Storage Foundation installed
