Server doesn't boot after creating new initramfs
Issue
- The Logical Volume (LV) with the root filesystem is no longer found at boot
- After patching the server it doesn't boot because the LVs where the OS is installed cannot be found.
- After creating a new initramfs the server doesn't boot because the LVs where the OS is installed cannot be found.
- Although the disk with the Volume Group (VG) containing the root LV is still visible within
rdshell, the VG is not found by issuingvgscan

- Red Hat Enterprise Linux (RHEL) 6
- Veritas
VxVMor Storage Foundation installed
- Veritas
