Issue

The Logical Volume (LV) with the root filesystem is no longer found at boot
After patching the server it doesn't boot because the LVs where the OS is installed cannot be found.
After creating a new initramfs the server doesn't boot because the LVs where the OS is installed cannot be found.
Although the disk with the Volume Group (VG) containing the root LV is still visible within rdshell, the VG is not found by issuing vgscan

Environment

Red Hat Enterprise Linux (RHEL) 6
- Veritas VxVM or Storage Foundation installed
