System log shows error - 'qla2xxx: Invalid completion handle' , F/W Error, and/or heartbeat failure events
Issue

One or more of the following events from the qla2xxx driver are present within the system logs
o Invalid handle events:qla2xxx [0000:2f:00.0]-5032:15: Invalid completion handle (5df) -- timed-out. qla2xxx [0000:2f:00.0]-5032:15: Invalid completion handle (5e0) -- timed-out. qla2xxx [0000:2f:00.0]-5030:15: Error entry - invalid handle/queue (2be5). qla2xxx [0000:2f:00.0]-5032:15: Invalid completion handle (408) -- timed-out.
o F/W errors and/or heartbeat failures:
qla2xxx [0000:2f:00.0]-5060:15: 83XX: F/W Error Reported: Check if reset required. qla2xxx [0000:2f:00.0]-5069:15: Heartbeat Failure encountered, chip reset required. qla2xxx [0000:2f:00.1]-5060:16: 83XX: F/W Error Reported: Check if reset required. qla2xxx [0000:2f:00.1]-5069:16: Heartbeat Failure encountered, chip reset required.

In-memory or on-disk corruption may be detected in conjunction with the above. events
o Either of these may cause filesystem corruption which is detected at shutdown, reboot, or via manual checks by running utilities such asfsck
.
