Emulex HBA taken offline after heartbeat timeout in Red Hat Enterprise Linux 5.4

Issue

  • Since upgrading to RHEL 5.4, we have been experiencing issues with the lpfc driver reporting the "0459 Adapter heartbeat failure, taking this port offline" and subseqently taking the HBA offline.  We have seen this issue on both our dev and test clusters, as well as on a standalone production machine.
  • Filesystem mounted read only when all paths go offline at the same time

Environment

  • Red Hat Enterprise Linux 5
    • Kernels between kernel-2.6.18-238.el5 earlier
    • lpfc module version 8.2.0.87.1p earlier(You can check lpfc driver version by using modinfo command as below):
      $ modinfo lpfc | grep description
      description:    Emulex LightPulse Fibre Channel SCSI driver [lpfc driver version]
      
  • Emulex HBA's including
    • Emulex LPe11K HBA (firmware version 2.82A3)
    • Fibre Channel: Emulex Corporation Zephyr LightPulse Fibre Channel Host Adapter (rev 02)
  • SAN models including
    • EMC SYMMETRIX with VMAX
