dmraid does not rebuild the isw mirror raid set (RAID1) after failure and replacement of a disk
Issue
- disk in isw dmraid setup was failed, replaced, but didn't rebuild correctly
- dmraid mirror rebuild not accepting new disk
- dmraid mirror rebuild failed to update status
- The disk in isw dmraid setup failed, rebuild failed
- disk replaced and rebuild done via the Intel Matrix Storage Manager option ROM bios utility at power on/boot time.
- after booting up the system, dmraid -s (status) is still
