Why are RHEV VMs pausing due to storage permission problems?

Issue

• Unable to resume the VM from paused state
• Other guests using the same storage are still operating normally
• This was a VM imported by virt-v2v from VMWare ESXi 5 several weeks ago
• The vm "intermed" has start successfully after the import and it has been running for several weeks without issue until now
• All 3 disks for this VM were imported
• No configuration changes after import
