VG losses disk

I am having a recurring problem with physical volume getting corrupted and dropping out of the volume group. I have Redhat 6
Kernel 2.6.32-431.11.2.el6.x86_64, LVM lvm2-libs-2.02.100-8.el6.x86_64. This is all running on Xenserver 6.02. I have now moved the data to a currently patch Redhat 6 so I am hoping there was a patch that fix this but I figured I should ask.

The setup is two x 2TB drives forming a 3,99TB logical volume. The first disk drops out of the VG with an error that it can not find the devices. I get the device back by recovering the PV (something like "pvcreate --uuid "FmGRh3-zhok-iVI8-7qTD-S5BI-MAEN-NYM5Sk" --restorefile /etc/lvm/archive/VG_00050.vg /dev/xvdb" then "vgcfgrestore VG") and it gets back in the VG. I am doing something wrong when recovering the VG so the recover PV is in a different location on the VG but that is a different issue. This seems to occur when the filesystem (using ext4) gets close to 100% fully.

Anyone else having this issue? Has it been patch?

Have you checked the Xenserver?
Does it keep representing the disk at the times the disk gets lost on the guest?
