Red Hat Enterprise Linux Fails to Boot After Upgrade From 5.4 to 5.7

Issue

  • When upgrading a RHEL 5.4 server running as a VM in VMWare to RHEL 5.7, the following errors occur:
Volume group "vg00" not found
Kernel panic - not syncing: Attempted to kill init!
  • All disks on Para Virtualized hardware controller are lost after upgrading the kernel in VMware guest on ESX server.

Environment

  • Red Hat Enterprise Linux 5
  • VMWare Hypervisor
  • Para Virtualized hardware using pvscsi
