Duplicated PV UUID on a virtual machine in RHEV

Issue

  • When attempting to create and add a new vDisk to a virtual machine, the disk was correctly created, attached to the VM and activated.
  • When attempting to create a new PV (pvcreate) on the new disk, it becomes apparent from LVS and VGS output that the disk already has a PV label and metadata.
  • The output contains data relating to a completely different VM. This VM appears to be one that recently had a snapshot removed.
