Issue

  • Both the mkfs.xfs and the mkfs.ext4 command stall on the ioctl() system call while trying to create a new file system on an lvm volume.
  • The machine appears to hang, and does not respond.
  • The /var/log/messages file has many SCSI entries like:
        kernel: sd 2:0:0:0: [sda] Sense Key : No Sense [current] 
        kernel: sd 2:0:0:0: [sda] Add. Sense: No additional sense information
        kernel: hv_storvsc vmbus_0_1: cmd 0x93 scsi status 0x2 srb status 0x6
