Red Hat Certificate System installation fails, pkicreate ends with an incomplete URL for the hostname

Issue

We are going through the installation and running pkicreate, following the instructions at

In section 6.2.1 - Installing and Configuring a CA.

After issuing the 'pkicreate' command in step 4, I am unable to access the config UI through FireFox.
It does not appear to be a firewall issue as the ports are open.
The browser's error is:

  The connection was interrupted
  The connection to 10.10.10.10:9445 was interrupted while the page was loading.

The tomcat and server logs did not yield any clues so far.
