Why does SPNEGO Fallback To Form Authentication not working on JBoss EAP?

I have SPNEGO authentication correctly configured and it works fine. I then use KB article "SPNEGO security with fallback to FORM user and password" and attempt to configure fallback authentication but it does not work. The fallback authenticator is never called based on my analysis of the log files.
