[RHOCP 4] Duplicate x-forwarded-host header value observed when application is integrated with Service Mesh

Issue

• When an application is integrated with OpenShift Service Mesh and accessed, the response contains a duplicate x-forwarded-host header values.
  Example:
  "x-forwarded-host": "api.hello.example.com,api.hello.example.com"
• The behavior may result in incorrect routing, failed authentication, or unexpected downstream behavior.
