Kdump fails with "smartpqi kdump init failed with error" message on console or fails to boot with "smartpqi controller is offline" message

Issue

• Kdump kernel gets reset by showing below messages in the serial console.

[   13.280008] smartpqi 0000:b6:00.0: controller is offline: status code 0x2e015
[   13.299097] smartpqi 0000:b6:00.0: kdump init failed with error -19

• Below are the messages before system reports IO error on multiple file systems at boot:
