First boot after new installation fails with post reboot error "/dev/sda1 0 UNEXPECTED INCONSISTENCY"

Issue

We have installed completely new server systems and each time the system does it's first POST install reboot there is the message: "/dev/sda1: UNEXPECTED INCONSISTENCY".

Once we run:

fsck /dev/vg00/lvroot
fsck /dev/vg00/lv_shared

the system will then boot normally and this message does not again.
