VM ephemeral disk corrupted after live migration

Issue

• VM ephemeal disk is corrupted after successfull live migration and VM can't boot anymore

• Impossible to mount /dev/vda is rescue mode and also impossible to repair xfs_filesystem

• Since RHSOP 16.2.1 upgrade, we have faced more than 10 VM with this problem.

• In emergency, we have resintalled those customer VMs. Some of them has been lost.

• We have tried to evacuate a compute node last Friday (the 3rd of June) related to another case but we have faced the same problem.

• The problem does not happened on all VMs. It seems to be random. There was only one migration at time when it happened.
  It happens on different compute nodes.

• It can't continue like that and we need Redhat to investigate quyickly whet is that oproblem and why it happens.
