Missing Smart Array cciss device links within /dev/disk/by-path

Issue

  • Not all cciss disks show up in /dev/disk/by-path
  • Missing cciss by-path links for devices
  • There are 2 cciss disks in /dev/disk/by-id but only 1 in by-path.
/dev/disk/by-id:
