LVM2 snapshot cannot be removed after attempting to merge an invalid snapshot in RHEL 6

Issue

If a snapshot becomes invalidated, and then a merge of that snapshot is attempted, the logical volume is no longer listed in lvs output but the extents are still shown as in use.
After merging an invalid snapshot, I cannot remove the snapshot
After merging an invalid snapshot, I cannot create another snapshot

Environment

Red Hat Enterprise Linux (RHEL) 6
lvm2 prior to release 2.02.95-10.el6 and/or device-mapper prior to release 1.02.74-10.el6
