EMC Clariion SAN LUNs intermittently failing

Issue

Different LUNs intermitently fail as below. A few are non-shared between hosts hostname1 and hostname2 and the rest are shared LUNs. Both hosts give the following output:

multipath -ll | grep failed
\_ 2:0:0:12 sdm 8:192 \[failed\]\[ready\]
\_ 4:0:0:6 sdy 65:128 \[failed\]\[ready\]
\_ 4:0:0:8 sdaa 65:160 \[failed\]\[ready\]
\_ 4:0:0:15 sdah 66:16 \[failed\]\[ready\]

There are also many path failures in /var/log/messages
