"multipath -d" shows mpath devices but they aren't created on Red Hat Enteprise Linux 5
Issue
- When the multipath -d command is run, it finds the device but states its in a [undef] state and therefore it isn't added when the command is actually run. I'm wondering how we could possibly instate this path and get it functioning again.

Environment
- Red Hat Enterprise Linux 5
- device-mapper-multipath root disk from SAN
- Attempting to use mdraid on top of device-mapper-multipath devices:
/dev/sda1 -| /dev/sdc1 -|=> /dev/mapper/mpath0p1 -| |==> /dev/md0 /dev/sdb1 -|=> /dev/mapper/mpath1p1 -| /dev/sdd1 -|
