Hot plugging SCSI disk leads to kernel panic with message: "kernel BUG at block/blk-core.c:1944"

Issue

Unplug a SCSI disk which is currently being accessed, then after a few minutes plug it back in again.

The host sometimes panics with the message:

kernel BUG at block/blk-core.c:1944.
