LogicalVolumes on mdraid devices backed by device-mapper-multipath LUNs are unresponsive

Issue

after the reboot not all MD-devices recovered correctly, the MD-devices consist out of device-mapper-multipath devices
reading from the filesystem on the LogicalVolumes on the MD-devices hangs indefinitely, processes go into D-state

Red Hat Enterprise Linux 5.3
SAN connected over device-mapper-multipath
- RAID1 mdraid on two device-mapper-multipath devices
  - LVM with filesytems on the mdraid devices
