mod_cluster does not remove persistently hung JVM or route traffic away from it
Issue
- One of our backend JVMs reached a permanently stalled state. All requests to it were always timing out and all periodic ping attempts from mod_cluster to it were timing out. But still, mod_cluster never removed this bad JVM from the balancer and it continued to attempt requests to it.
