Why are no ceph-mgr standby instances running in an OpenShift Container Storage 4.x cluster?

Issue

Red Hat OpenShift Container Storage installs only one (1) ceph-mgr Pod by default.
From my general experience with "Ceph", the number of ceph-mgr processes should be at least three (3) to not have a single point of failure and to have "standby" ceph-mgr processes running.
How to properly add additional ceph-mgr Pods in Red Hat OpenShift Container Storage?
