System installation via USB key fails with "kernel: sdw: unknown partition table" errors or "error in cmd completion:>
Issue
During the installation of redhat 7, it fails with errors such as:

And/or:
scsi host: error in cmd completion : Subsystem : 1 Opcode : 191 status(compl/extd)=2/30
