[RHEL5.5.z] panic on boot attaching scsi_dh hardware handler with EMC active/active Clariion storage

Issue

  • Kernel panic on boot with active/active Clariion storage.
  • Kernel panic on boot with messages indicating in kfree called from scsi_dh:scsi_dh_attach
  • Kernel panic on boot with messages indicating scsi_dh:scsi_dh_handler_attach

Environment

  • Red Hat Enterprise Linux 5.5   
    • kernel-2.6.18-194.26.1.el5
  • Clariion active/active ALUA, failover mode 4.   
    • Problem does not occur when failover mode is set to 1.
  • EMC PowerPath, version - 5.5.0.00.00-275   
    • Powerpath loaded on boot as well as dm-mp
  • EMC NaviHostAgent - 6.29.0.6.1-1
