Device not responding after hpsa driver issued a target reset

Issue

  • /dev/sdb device stopped responding to lvm commands.
# pvs |grep -i error
  /dev/sdb2: read failed after 0 of 512 at 317273604096: Input/output error
  /dev/sdb2: read failed after 0 of 512 at 317273714688: Input/output error
  /dev/sdb2: read failed after 0 of 512 at 0: Input/output error
  /dev/sdb2: read failed after 0 of 512 at 4096: Input/output error
  /dev/sdb2: read failed after 0 of 2048 at 0: Input/output error
  • The following errors can be seen in the output of 'dmesg':
hpsa 0000:02:00.0: resetting device 2:0:0:2
hpsa 0000:02:00.0: cp ffff88003784e000 is reported invalid (probably means target device no longer present)
hpsa 0000:02:00.0: resetting device failed.

  • hpsa driver.
