Why the system fails to boot after replacing a disk in Hardware RAID array?

Issue

  • After replacing a failed disk in the RAID array, system fails to boot
  • How to rebuild the Hardware RAID array after one of the failed disks have been replaced?
  • System boots up fine if the new disk is removed from the RAID array(mirror), but fails to boot if both the disks are attached in the array.
  • While booting up system fails to obtain some of the files required to boot the system.
