After an upgrade from 6.3 to 6.4 the system errors out with 'dracut Warning: LVM vg00/swap1 not found'
Issue
After updating our server from version 6.3 to 6.4 with yum update the machine kernel panics after every reboot. One of the warnings we see is the following:
dracut Warning: LVM vg00/swap1 not found
