Multipath devices missing after kernel upgrade

Issue

  • After upgrading the kernel, multipath devices are missing
# multipath -ll
#
  • After upgrade /boot is no longer mounting
# ls -l /boot
total 0
# mount | grep boot
#
  • After upgrade, boot is failing fsck at boot time and machine falls into maintenance mode
*** An error occurred during the file system check.
*** Dropping you to a shell; the system will reboot
*** when you leave the shell.
Give root password for maintenance
(or type Control-D to continue):

  • Red Hat Enterprise Linux 5
  • Boot from SAN
  • / on multipath device
