Runaway I/O on SAN disk using RHEL with EMC PowerPath

Issue

IO appears to be 100% on SAN disk and makes the device inaccessible. sda/sdb are two paths to the same vxfs filesystem.

iostat -x 2 output:

sdb               0.00     0.00  0.00  0.00     0.00     0.00     0.00   182.09    0.00   0.00 100.05
