OCS Heketi can't start, or some Volumes can't be mounted, or some Bricks are offline during installation or after a Node reboot

Issue

Heketi can't start during an OCS installation or after a Node reboot.

Heketi is a dynamic storage provider for OCS. It creates and removes volumes inside of a Gluster cluster running in either converged mode (in containers) or in independent mode (using systemd gluster daemons)

When you have OCS in converged mode, Heketi is installed together with OCS using openshift-ansible playbooks.

or

Gluster volume status returns Bricks offline and Volumes are in read-only or not mounting.
