clvmd segfaults on startup in RHEL 5 Update 5 after error "WARNING: Ignoring duplicate config node: types (seeking types) "

Issue

Trying to start clvmd on a cluster prints out a warning message about the configuration file and then crashes.

On the console:

Starting clvmd: [ OK ] 
Activating VGs: WARNING: Ignoring duplicate config node: types (seeking types) 
connect() failed on local socket: Connection refused 
WARNING: Falling back to local file-based locking. 
Volume Groups with the clustered attribute will be inaccessible. 
5 logical volume(s) in volume group "volgrp01" now active 
[ OK ]
