System gets hung during the IO on disks connected to Adaptect Smart Storage controller

Issue

• System gets hung during the IO on disks connected to Adaptec Smart Storage controller.
  At the time of hang, there are following errors logged on console:

  sd 2:0:5:0: Power-on or device reset occurred
  smartpqi 0000:b1:00.0: resetting scsi 2:0:5:0
  smartpqi 0000:b1:00.0: reset of scsi 2:0:5:0: SUCCESS
