Unable to Remove Duplicate OpenShift Providers

Issue

There are duplicate OpenShift providers attached to CloudForms (same name, same hostname, same environment). They were added via an API request
When trying to remove the provider via the Web UI or via a Rails command, the following error is reported:

ActiveRecord::RecordInvalid: Validation failed: Name has already been taken, Host Name has to be unique per provider type
