Oracle voting disk is taken offline around the time "SCSI error: return code = 0x000e0008" is logged

Issue

  • /var/log/messages reports the similar to the following

    sd 4:0:0:42: SCSI error: return code = 0x000e0008

  • and in the Oracle logs, one (or more) voting disks is considered not functional:

    [cssd(20024)]CRS-1613:No I/O has completed after 90% of the maximum interval. Voting file ORCL:LUN001 will be considered not functional in 19180 milliseconds
