Issue

/var/log/messages contains following messages:
```

Nov 30 13:08:36 hostname kernel: sd 0:0:0:0: [sda]  Add. Sense: Defect list not found
```
- Note that no I/O command line is included in the event, that is there is no lines similar to the one below. This indicates that the command generating these events is likely a non-data movement command issued via ioctl request.
