Issue

  • Filesystems went to read only mode after following error:

Environment

  • Red Hat Enterprise Linux 6
    • Observed on kernel-2.6.32-279.22.1.el6.
    • Issue began to occur after upgrading from kernel-2.6.32-279.9.1.el6.
  • dm-multipath
  • Dell Poweredge R710 with Dell PERC H700 integrated storage controller:
    • Observed on storage controller firmware version 2.10.
    • PERC H700 controller is on the latest firmware.
