Ceph - OSD's are listed as osdN instead of deviceN, under the devices tag in CRUSH map.

Issue

• OSD's are listed as osdN instead of deviceN in CRUSH map.
• Wrong device osd name in CRUSH map after replacing a failed disk.

• Following entries get inserted in CRUSH map.
