How can a custom http header be logged in access logging of Apache httpd?

Solution Verified - Updated -

Issue

There are cases when additional httpd headers need to be logged in Apache logs. This is very useful in cases where requests are proxied from one web server to another and additional information is added on the proxy (i.e. real source IP).
