FC path recovery on KVM guest side

Solution Verified - Updated -

Issue

When a FC cable gets disconnected, both Host and KVM guest identify the path failure.
When the connectivity is restored, the Host recognizes that the path is active, but KVM guest does not, so the path is in status failed.
I/O operations to the previously failed path do not work (using dd returns input/output error).
KVM guest/domain connectivity to the LUN via the affected path is never restored until the domain is destroyed and started again.
Simple reboot of the domain does not help.

Environment
