Why do QLogic HBA's using qla drivers sometimes show messages "RISC paused -- HCCR=0, Dumping firmware!"?

Solution Verified - Updated -

Issue

  • The HBA card is reset automatically.
  • LIP reset occuring.
  • There are a few error messages in /var/log/messages.

    • Following messages were found in the vmcore file's log:

qla2xxx 0000:12:00.0: Firmware dump saved to temp buffer (3/ffffc20010347000).
qla2xxx 0000:12:00.0: Performing ISP error recovery - ha= ffff81202d2504f8.

    • kernel-2.6.18-128.7.1.el5
    • qla2xxx driver version: 8.02.00.06.05.03-k
  • QLogic HBA (including following models):
    • Fibre Channel: QLogic Corp. ISP2432-based 4Gb Fibre Channel to PCI Express HBA (rev 03)
